2020/05/29(金) 06:00:37.62ID:kpWhJiVR0厚生労働省の薬食審・医薬品第二部会は5月28日、Web会議で新薬6製品の承認の可否を審議し、いずれも承認することを了承した。
この中には、ICS/LAMA/LABAの3成分を配合した気管支喘息薬エナジア吸入用カプセルがある。
ICS/LAMA/LABA配合薬は慢性閉塞性肺疾患(COPD)の適応で承認されている薬剤はあるが、エナジアが正式に承認されれば、喘息適応の3成分配合薬は同剤が初となる。順調にいけば6月に正式承認される。
【審議品目】
▽エナジア吸入用カプセル中用量、同カプセル高用量(インダカテロール酢酸塩／グリコピロニウム臭化物／モメタゾンフランカルボン酸エステル、ノバルティスファーマ)：
「気管支喘息(吸入ステロイド剤、長時間作用性吸入β2刺激剤及び長時間作用性吸入抗コリン剤との併用が必要な場合)」を効能・効果とする新有効成分含有医薬品および新医療用配合剤。再審査期間は８年。
長時間作用性β2刺激薬(LABA)のインダカテロール酢酸塩が新有効成分となる。既承認成分のグリコピロニウムは長時間作用性抗コリン薬(LAMA)、モメタゾンは吸入ステロイド薬(ICS)。
同剤はICS/LAMA/LABAによる3成分配合吸入薬。１日１回、専用の吸入用器具を用いて吸入する。
2規格ともインダカテロール150μg、グリコピロニウム50μgを含有する。そして、同カプセル中用量はモメタゾン80μg、同カプセル高用量はモメタゾン160μgを含有する。
COPD適応のICS/LAMA/LABA配合吸入薬はビレーズトリエアロスフィアとテリルジーがあるが、気管支喘息適応ではエナジアが初となる。
エナジアは、海外で承認されている国・地域はない。

Announced today the publication of data from its Phase I liver cancer trial, OUTREACH, in Clinical Cancer Research. It is the first publication in which a small activating RNA treatment (MTL-CEBPA) demonstrated clinical benefit.
Publication and OUTREACH Study Update:
The publication in Clinical Cancer Research summarizes the results from MiNA’s Phase I, open-label, dose escalation and dose expansion trial of MTL-CEBPA, OUTREACH, in adults with advanced Hepatocellular Carcinoma (HCC).
Overall, MTL-CEBPA was well-tolerated and demonstrated pharmacodynamic target engagement, meeting the primary endpoint of the study.
Furthermore, a reduction of suppressive immune cells in the tumour microenvironment as well as initial signs of potential synergistic efficacy when combined with standard of care tyrosine kinase inhibitors in HCC could be observed.
These encouraging Phase I data validate the targeting of C/EBP-α as a novel therapeutic strategy in cancer and prompted a Phase Ib study further evaluating MTL-CEBPA in combination with sorafenib in HCC.
Enrolment for the Phase Ib part of the OUTREACH trial was completed in Q1 2020 and initial results will be presented during a poster session at the forthcoming American Society of Clinical Oncology (ASCO) on Friday, May 29, 2020.
The framework for a subsequent Phase II clinical trial is currently being designed with the objective of initiating this next stage of clinical development in the second half of 2020.

LONDON--MiNA Therapeutics, the pioneer in RNA activation therapeutics, announced today top line results from the Phase Ib dose escalation and cohort expansion study,
OUTREACH, of lead candidate MTL-CEBPA in combination with sorafenib standard of care in patients with advanced hepatocellular carcinoma (HCC or liver cancer).
The study met its primary endpoints of safety and tolerability for MTL‑CEBPA administered either concomitantly or sequentially with sorafenib.
In addition, five patients experienced objective tumour responses, including two complete responses during the combination treatment.
The data will be presented during a poster session at the 2020 American Society of Clinical Oncology (ASCO) Annual Meeting, held virtually from May 29 - May 31, 2020.

At the data cut-off of February 1, 2020, 36 patients with advanced HCC had been treated with MTL‑CEBPA in combination with sorafenib in the Phase Ib study.
22 patients received MTL‑CEBPA and sorafenib concomitantly, and 14 patients received the two agents sequentially.
Both concomitant and sequential treatment regimens were generally very well tolerated, and no maximum tolerated dose was determined.
The profile of adverse events was consistent with the known safety profile of each agent and the underlying disease.
In addition, concomitant sorafenib treatment did not alter the pharmacokinetics of MTL‑CEBPA.
Five patients who were naïve to prior tyrosine kinase inhibitor (TKI) treatment experienced objective tumour responses, including two patients who experienced complete remission.
Tumour responses were most pronounced in those TKI naïve patients with viral aetiology, where four out of nine evaluable patients experienced objective responses.

These encouraging Phase Ib data add to the previously published positive Phase I results in which four out of five patients experienced a durable, objective response to off-study sorafenib treatment after discontinuation of MTL‑CEBPA.
As a single agent treatment, sorafenib is associated with a very low objective response rate.
In a recent Phase III study, complete responses were observed in 1% of patients and partial responses were observed in 6% of patients based on RECIST 1.1 criteria in 372 patients1.

[Abstract 5518]: "A Phase I, open-label, multicenter study to assess the safety, pharmacokinetics, and preliminary antitumor activity of AZD4635 both as monotherapy
and in combination in patients with advanced solid malignancies: Results from prostate cancer patients".
Extracellular adenosine accumulates in hypoxic tumor environments and is associated with decreased immune response.
This immune suppression is thought to be in part due to adenosine binding to the T-cell adenosine 2A receptor (A2aR).
There is interest in targeting the A2aR based on data showing A2aR upregulation in response to treatment with the anti-PD-L1 antibody durvalumab,
and data showing enhanced T-cell activation with combination PD1/PDL1 and A2aR axis therapy.

1日の東京株式市場で、新興企業株を反映する東証マザーズ指数は1000ポイントを上回った。大台に乗せたのは2018年12月以来。
新型コロナウイルスが流行し景気の先行き不透明感が残る中で、新興企業の成長力を評価する動きが強まっている。
マザーズ指数の大台回復には、新型コロナウイルスのワクチン開発を進めるアンジェスが4月から急騰して大きく貢献した。
時価総額の大きいメルカリやそーせいグループのほか、昨年12月上場のフリーやメドレーも指数を押し上げた。
3月安値からの上昇率は約9割で、3割程度にとどまるTOPIXなどに比べて上げが顕著だ。
リブラ・インベストメントの佐久間康郎社長は「限られたテーマ株が散発的にあるような通常の状態と違い、
現在はバイオやオンライン、AIなど幅広いテーマで活発に物色されているのが特徴だ」と語る。
新型ウイルスの影響で東証1部は半年後、1年後の業績回復の角度が緩慢と予想される一方、「新しいサービス業が多いマザーズ企業はＶ字回復の蓋然性が高い」とみる。
こうした背景には「日本のナスダック」としての評価がある。

大和証券投資情報部の石黒英之シニアストラテジストは「マザーズは全体の4分の3がバイオやITなどの成長産業。
製造業などと異なりコロナの悪影響を受けないばかりか、むしろ治療薬・ワクチン開発やIT投資活発化と時代の変化を取り込める」と評価。
マザーズは「産業構造の面からは米ナスダック総合指数に似ている」と言う。
米国でもS&P500種株価指数やダウ工業株30種平均に比べ、大型テクノロジー株やバイオ株の比重が相対的に高いナスダック総合指数は戻りが大きい。
リブラの佐久間氏は「数年に一度のスケールの相場。1000ポイントは単なる通過点で、中期的には18年高値（1300台）を目指す」と予想する。
もっとも、マザーズ市場は個人投資家の多くが参加する市場だけに目先資金が出入りしやすい。
指数も2カ月近く一本調子で上昇し、短期的な過熱感を指摘する声もある。
SMBC信託銀行の佐溝将司マーケットアナリストは「在宅勤務が増えて個人投資家のFX取引が増えていると言われている。
マザーズ銘柄のような中小型株にも似たような傾向があるのではないか」と話した。

[Abstract 5518]: "A Phase I, open-label, multicenter study to assess the safety, pharmacokinetics, and preliminary antitumor activity of AZD4635 both as monotherapy
and in combination in patients with advanced solid malignancies: Results from prostate cancer patients".
Extracellular adenosine accumulates in hypoxic tumor environments and is associated with decreased immune response.
This immune suppression is thought to be in part due to adenosine binding to the T-cell adenosine 2A receptor (A2aR).
There is interest in targeting the A2aR based on data showing A2aR upregulation in response to treatment with the anti-PD-L1 antibody durvalumab,
and data showing enhanced T-cell activation with combination PD1/PDL1 and A2aR axis therapy.
This abstract presented data as part of a larger multi-center Phase I trial in multiple cancers looking at AZD4635 monotherapy or in combination with specific agents across multiple cancer types.
The trial utilizes a Bayesian logistic regression model-based approach in its design.
In prostate cancer patients, AZD4635 was administered as monotherapy or in combination with durvalumab.
The results from a cohort of heavily pretreated prostate cancer patients (median five prior treatments) showed response in a subset of patients
and suggest that progression-free survival was prolonged in patients with evidence of higher adenosine-associated gene expression relative to those lacking this signature.

Ana Aparicio, MD, provided her perspective on these emerging promising therapeutic targets in prostate cancer.

In the second abstract, Emerson Lim, MD, and colleagues presented the prostate cancer subgroup of a phase 1 trial assessing AZD4635 in advanced solid cancers.
AZD4635 is an inhibitor of adenosine 2a receptor signaling which improves immune activation and anti-tumor activity in preclinical models.
In this subset of the phase I trial, patients with refractory metastatic castration-resistant prostate cancer received either AZD4635 monotherapy (at two different doses) or AZD4635 in combination with durvalumab.
In a non-randomized fashion, 49 patients received monotherapy and 45 patients received combination therapy.
The majority had good performance status and median age was 70.5 years.
These patients had extensively pre-treated disease with a median number of prior treatment regimes of 5 (range 1-10),
wit the majority having receiving prior chemotherapy (61%) and an even larger proportion having received androgen axis inhibitors (90%).
Of 70 patients available for assessment, confirmed objective response rates were 6.1% in the monotherapy arm and 16.2% in the combination therapy arm.
5 patients had ongoing responses with a duration between 1 and 18.5 months.
A high adenosine gene expression signature was associated with longer progression-free survival among treated patients.
Adverese events including nausea, vomiting, fatigue, decreased appetite, dizziness and diarrhea.

Finally, with respect to AZD4635, Dr. Aparicio highlighted the mechanistic data highlighting the synergistic effect of adenosine mechanisms and immunotherapeutic approaches.

Abstract:
The short chain fatty acids (SCFAs) acetate, butyrate and propionate, are produced by the fermentation of non-digestible carbohydrates by the gut microbiota.
SCFAs are of interest because they regulate appetite, adiposity, metabolism, glycemic control and immunity. SCFAs act at two distinct G protein coupled receptors (GPCRs), FFAR2 and FFAR3.
These are expressed in intestinal enteroendocrine cells (EECs), where they mediate SCFA-driven anorectic gut hormone release.
EECs also express other GPCRs that act as nutrient sensors, in a manner that is plastic and adaptable to the environment.
SCFAs may elicit some of their health-promoting effects by altering levels of GPCRs in EECs, thus, enhancing gut sensitivity to dietary molecules.

Interestingly, treatment of EECs with propionate or butyrate, but not acetate, increased levels of umami receptor transcripts.
This phenomenon was reversed by inhibiting Gαi/o signaling with pertussis toxin, suggesting that SCFAs act through FFAR2/3 to alter gene expression.
Surprisingly, neither a FFAR3 nor a FFAR2 selective synthetic ligand could increase TASR1/TASR3 mRNA levels.
We assessed the functional impact of increases in TASR1/TASR3 expression using unique pharmacological properties of the umami taste receptor; namely, the potentiation of signaling by inosine monophosphate.
We found that the umami taste receptor induced inosine 1 phosphate and calcium signaling in response to L-alanine and L-monosodium glutamate,

AstraZeneca社は、進行性前立腺がんの次世代免疫療法として開発中のAZD4635の単剤、およびPD-L1抗体デュルバルマブ(Imfinzi)との併用の第1a相試験における新しい臨床データを
2020 ASCO Virtual Scientific Program(2020年5月29〜31日)で発表しました 。
AZD4635はSosei Heptaresが創製し、2015年にAstraZenecaに独占的ライセンス供与された経口可能なアデノシン2A受容体(A2aR)拮抗薬です。
アデノシンシグナル伝達は、免疫回避メカニズムとして腫瘍によって組み込まれた自己免疫を回避する通常のプロセスです。
A2aRを介したシグナル伝達の薬理学的遮断は、アデノシンの免疫抑制効果を妨げ、免疫チェックポイント阻害薬の併用で、より強力な抗腫瘍免疫応答を可能にします。

and in combination in patients with advanced solid malignancies: results from prostate cancer patients (NCT02740985),” and reported the following:
・In patients with advanced prostate cancer (mCRPC), AZD4635 alone or in combination with durvalumab was tolerable and associated with clinical benefit.
・Retrospective analysis of Progression-Free Survival (PFS) shows that the clinical benefit is associated with high adenosine (ADO) gene expression signature in peripheral blood.
・Analysis of patient peripheral blood provides evidence of immune activation.
・Additional exploratory analysis of circulating tumor DNA (ctDNA) demonstrated several key altered pathways, which suggest potential combination strategies.
・Additional studies are ongoing for AZD4635 in mCRPC and other tumor types.
